360' appraisal

I would like to do a 360' appraisal for my own benefit can anyone help - I have looked at the following resources but none really seem totally fit for purpose - I am a GP in a medium size group practice

ah seem to have solved this have found that since the white paper there are at least 3 companies offering this commercially one of which has "done" consultants and that the GMC will issue a statement on this shortly - however must be in your folder every 3 years (£75 I hear)
